Question 39 - Revocation
This question requires you to provide information regarding revoked credit cards or charged accounts that have not been resolved by bankruptcy. A statement from creditors showing proof of payment must be uploaded for each revoked account.
Question 40 - 42 - Defaulted Student Loan/Other Defaulted Debt/Past Due Debt
This question requires whether or not you have defaulted on a student loan. If there was a defaulted student loan or any other debt, you will need to upload proof of payment or current balance from each creditor.
If you have ever filed a petition for bankruptcy this question requires you to provide the date and type of bankruptcy. You will need to upload associated schedule of indebtedness, petition, docket report, and discharge from bankruptcy order. If you need to find old bankruptcy records you can search PACER to access court records or review a local court website to see if you can gain access there.
